救急車の出動回数を減らす

ゆみのハートクリニックの社会における使命のひとつとして、
「救急車の出動回数を減らす」がある。

昨年まで、総合病院や大学病院で心臓専門医として従事していた際、
救急車を利用して、結果的に救急対応の必要がない患者を数多く見てきた。

最近の消防庁の統計では、全国の救急車の出動件数が、
2012年に約580万件と過去最多になり、その結果、患者搬送時間も増加している。
救急車搬送患者の重症度をみると、
その50%以上は軽症にすぎず、重症は10%程度であったとも言われる。

ゆみのハートクリニックでは、心臓病を専門として、
外来から在宅訪問診療、緊急往診を行っているが、
患者さんに何か変化があったときは、救急車ではなく、
まずクリニックへ連絡をいただき、
可能な限りの初期対応を行うことを使命として考え、実行している。

心臓病の行く末でもある心不全という病態は、
末期がん患者とは違い、病状は波を打つように
増悪と軽快を繰り返しながら、徐々に進行していく。
そのため、心不全の増悪傾向をいち早く察し、
その増悪を抑えることが必要である。

毎日、若干名ほどの緊急での往診対応ではあるが、
それにより、救急車コールによる救急車への負担、
総合病院や大学病院への患者対応負担、
そして患者・家族の身体的、精神的、経済的負担などを軽減することができる。
それを私たちの社会的使命として、これからも診療を続けていく。

Our clinic's mission to decrease the number of ambulance dispatch

One of our clinic's mission for the society is to "decrease the number of ambulance dispatch."

When I used to work as a cardiologist in university hospital, I had encountered many cases which did not require emergency care.

A recent statistic from the fire department reports in Japan that the number of ambulance dispatch reached close to 5.8 million times in 2012, which is all time high. At them same time, the time required to take the patient to a hospital is increasing. Also, there exists a data that looked into the severity of those transported patients showing that only 10% were severe and needed emergency care while over 50% were mild.

As cardiology specialist at Yumino Heart Clinic, we provide outpatient, home healthcare, and home visiting care. Every member of our clinic believes that it is our responsibility to take the first action as much as possible before calling emergency services when a patient requires medical care.

Heart failure, which is the last stage of heart disease, is different from the terminal stage of cancer as heart failure patients go through exacerbations and improvements during the progression of the disease. Therefore, it is critical to catch the symptoms of exacerbation and take necessary actions beforehand.

Everyday, we provide emergency home visiting care to few patients believing that our actions are reducing the burden to ambulance dispatches, to healthcare professionals in general or university hospitals. Moreover, patients and their family members do not need to suffer unnecessary mental and economic burdens. We find great values and responsibilities in our actions to the society and would want to keep them in our mind as we continue to provide care with this in our mind.
