About YUMINO
About YUMINO
Yumino Medical Corporation, established in 2012, is a comprehensive general internal medicine clinic group providing integrated outpatient and home-based medical care. We operate eight locations across Tokyo, Osaka, Ehime, and Fukuoka, as well as two locations in Vietnam. With the vision of "a society in which hospitalizations other than for surgery are eliminated by 2030," we deliver community-based medical care that supports each individual's way of life through outpatient services, home-visit medical care, home nursing, home rehabilitation, and home nutrition guidance. By 2025, we manage medical care for approximately 5,000 home-care patients, providing advanced and comprehensive treatment in their own homes for patients who have difficulty visiting clinics and who require a high level of medical support. Our organization employs approximately 500 staff members, including around 120 physicians. Our Chairman, Dai Yumino, serves as a Board Member of the Japanese Circulation Association and as Secretary General of the National Cardiology Clinic Network of Japan.

- Patients under home medical management: 4,272
- Home nursing service users: 244

Message from the Chairman
As a cardiology specialist, I have cared for patients with heart failure--often the final stage of heart disease--at university hospitals in Tokyo. Heart failure is a progressive condition in which symptoms worsen in daily life, leading to repeated hospitalizations and a gradual decline in a patient's independence.
Despite providing the most advanced medical treatment to hospitalized patients, there were many times when we were unable to meet the wishes of patients and their families. This experience was deeply distressing.
I came to realize that in order to prevent readmission and maintain quality of life, medical care must be delivered closer to patients' daily lives--not in hospitals alone, but within the community, in a way that is best suited to each individual. With this belief, I established a clinic that provides both outpatient and home-based medical care.
"LIFE" means not only life itself, but also daily living and one's entire life journey.
Through caring for many patients in community settings rather than hospitals, I have learned that medicine is not merely about prolonging life. It is about understanding each person's way of living and人生, and asking how we can best support a life that is truly their own. To support each individual's unique LIFE, we must maintain a high level of medical expertise, continually ask what is truly best for each patient, and work together as a team to seek those answers.。
With sincere passion and a strong sense of social responsibility, we will continue to be a medical corporation dedicated to supporting lives that are true to each individual.。

Profile
After graduating from Juntendo University School of Medicine, I joined the Department of Cardiology at Tokyo Women's Medical University Hospital. Through my clinical experience, I became acutely aware of the need for additional home-based treatment and care for patients with advanced heart failure. I subsequently studied at the University of Toronto in Canada, where I conducted research on home management of heart failure. This work was recognized with Young Investigator Awards from both the American College of Cardiology and the American Thoracic Society. After returning to Japan, I continued clinical practice and research focused on patients with severe heart failure and went on to establish Yumino Heart Clinic. As the Chairman of Yumino Medical Corporation, I am committed to clinical care, education, and research, striving to build a society in which people living with heart disease and other disabilities can live safely and with peace of mind.
